General Description

Under the direction of the Director of Nursing, the RN Unit Manager is responsible for maintaining the standards of nursing care and practices of up to 60 residents while meeting the physical, psychosocial, and rehabilitation needs of the residents.  The RN Unit Manager is also responsible for the daily management of LPNs and CNAs on the unit while coordinating care, performing assessments, and adhering to documentation guidelines. 

DUTIES
JOB KNOWLEDGE AND ROLE RESPONSIBILITIES:
1.Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning delivery of care.
2.Knowledge, skills, and techniques necessary to care for residents with the following needs: alzheimers, dementia, dialysis, IV therapy, infectious diseases, wound care, and all disease processes if applicable.
3.Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained.
4.Makes daily rounds for all areas of nursing care under his/her supervision, visits each resident to evaluate physical and emotional condition to receive comments relating to the resident's needs and problems, and implements necessary nursing interventions.

General Description:

Under the direction of the Director of Nursing, the RN Shift Supervisor / Relief Supervisor is responsible for maintaining the standards of nursing care and practices within the facility to meet the physical, psychosocial, and rehabilitation needs of the residents.

Qualifications:

Registered Nurse with current MS license, and no restrictions. Must speak English; have computer skills; LTC experience, and excellent documentation skills.

Essential Duties:

1. Perform various duties to provide quality nursing care to residents to maintain or attain the highest practical level of functioning and to coordinate total nursing care for the residents as illustrated by the following:

2. Assesses residents who have had an accident or incident that results in injury or any unusual event; documents findings as per Resident Care Manual Policy.

3. Thoroughly investigates allegations of alleged abuse / neglect or misappropriation of property and notifies appropriate parties as per policy.

4. Makes daily rounds in areas of nursing care under his/her supervision, visiting each unit to evaluate overall unit performance, and to receive comments relating to the residents needs and problems, and implement necessary changes.

5. Delegates responsibility to nursing personnel through work assignments, etc. for direct nursing and non-nursing care of patients during the shift.

6. Assists in assessing level and number of personnel assigned to the unit. Makes recommendations regarding such personnel where indicated for better patient care and utilization of such personnel.

7. Supervises and evaluates the performance of nursing personnel under his/her direct supervision.

8. Instructs nursing personnel on new policies and procedures and treatments to allow for the most effective use of nursing time.

9. Reports status of nursing personnel and other supervisory matters to the Assistant Director of Nursing Services and/or Director of Nursing Services, on a daily basis.

10. Makes recommendations for hiring, disciplining, evaluating and the termination of personnel through written reports and verbal communication.

11. Completes appropriate evaluation summaries and assists with those completed by Unit Managers and/or Charge Nurses, as needed.

12. Assists in the scheduling of personnel to adequately cover all nursing shifts.

13. Assists with the preparation of the nursing department budget and adheres to that budget as approved by the Executive Director.

14. Assesses, monitors, and treats specific patient care conditions pursuant to physician orders.

15. Maintains open line of communication with attending physician regarding patient care and/or treatment issues.

16. Verifies the coordination of nursing care with other departments and disciplines within the Facility to best meet resident care objectives.

17. Conducts periodic rounds to observe Facility appearance and safety, and makes recommendations or assignments for nursing personnel in regard to these concerns and/or report these concerns to the appropriate personnel.

18. Coordinates the resolution of resident activity, social service, and nutritional problems observed by nursing personnel.

General Description

Under the direction of the Director of Nursing Services or SDC, performs recruitment and staffing in accordance with Facility policy and procedures. Essential Duties 

1. Maintains a nursing department work schedule that adheres to the Facility’s staffto-resident ratio.

2. Administers Facility work rules, policies and procedures and collective bargaining agreements (if applicable) in the scheduling of staff.

3. Reduces unscheduled vacancies by contacting available staff to cover shifts.

4. Completes and posts daily schedule per state regulations.

5. Supports adherence to Facility attendance, paid time off, leave of absence and FMLA policies.

6. Schedules and conducts interviews with internal and external nursing department candidates as needed.

7. Verifies absence/tardy forms are completed and forwarded to Human Resources in a timely manner.

Brief Job Summary for Registered Nurse Infection Control Nurse

Under the direction of the Director of Nursing Services, the Infection Preventions serves as a support person within the facility, providing guidance and education; assistance in problem solving related to resident care; monitoring compliance with state and federal regulations and coordinates the Infection Prevention and Control Program as set forth in the Resident Care Policy and Procedure Manual.

Provides directive and supportive consultation to facility staff.

Partners with the Medical Director, Executive Director and Director of Nursing to develop, implement, and evaluate annual infection prevention goals and plan.

Oversees the Infection Prevention and Control Program including surveillance of healthcare acquired and community acquired infections

Conducts infection surveillance and reports routinely to the Quality Assurance Committee

Directs and assists in preparing reports, statistical data, and trend identification for the QA committee.

Works with facility for problem solving when needs arise in areas such as:

Monitoring resident care delivery systems

Performing chart reviews

Checking Physician Orders as needed

Provides education and training regarding infection prevention and control practices to all staff. Includes evidence-based infection control practices and isolation precautions.

9. Reviews department policies, procedures, manuals, job descriptions, etc., at least annually and participates in making recommended changes.

10. Assists in the development / monitoring / maintaining of the department budget.

11. Develops and participates in programs designed for in-service education, on the job training, and orientation classes for newly assigned department personnel.

12. Assists in the recruitment, selection, and training of a Director of Food & Nutrition Services, as requested.

13. Involves the resident / family in planning objectives and goals for the resident.

14. Reviews and edits menus as prescribed by the Diet Manual.

15. Writes and approves therapeutic diets not defined on the cycle menu for which there are physician orders, instructs the staff on proper service of diets

16. Inspects the food service area and practices for compliance with current applicable regulations.

17. Serves on various committees of the Facility as appointed by the Executive Director.

8. Documents on high risk residents monthly.

Qualifications Education/Training/Experience: Must be a qualified dietician or other clinically qualified nutrition professional either fulltime, part-time or on a consultant basis. A qualified dietician or other clinically qualified nutrition professional is one who - i. Holds a bachelor’s or higher degree granted by a regionally accredited college or university in the United States (or an equivalent foreign degree) with completion of the academic requirements of a program in nutrition or dietetics accredited by an appropriate national accreditation organization recognized for this purpose. ii. Has completed at least 900 hours of supervised dietetics practice under the supervision of a registered dietician or nutrition professional. iii. Is licensed or certified as a dietician or nutrition professional by the State in which the services are performed. In a state that does not provide for licensure or certification, the individual will be deemed to have met this requirement if he or she is recognized as a “registered dietician” by the Commission on Dietetic Registration or its successor organization. iv. For dieticians hired or contracted with prior to November 28, 2016, meets these requirements no later than 5 years after November 28, 2016 or as required by state law.
